I would like to bring to your attention to a cause that is close to my heart. As you know I have organized a few fundraisers for the Montreal Canadiens Children Foundation, and so i thought this would be ther perfect place to upload the pdf of the donation form for this wonderful cause.

The Montreal Canadiens Children's Foundation helps underprivileged, sick and special-needs children by contributing financial resources in the areas of poverty, health, well-being and amateur sport. Since its inception back in August 2000, the Foundation has donated some $6 million to over 250 charitable endeavors from across the province of Quebec.
